Barcelona Hills
2008- 09 School Year New Student Registration
Parents are invited to pick up and return registration forms during regular office hours.  See below for required check-out information.

Q:  What is required to register my child? 

A:  Parents/guardians will need to complete the various CUSD and Barcelona Hills registration forms. To check-out a registration packet, parents must present: 

• Child's Immunization Record (yellow card)

• Two proofs of residence (utility bills and/or a lease agreement)

• Birth certificate
  (kindergarten & first grade)

• Record of physical exam (kindergarten & first grade)

• Record of a current dental exam (kindergarten & first grade).  

All completed forms are needed in the school office at least 2 days before a new student is scheduled to begin class.  Please visit our Elementary Clerk, Debbie Johnson, or e-mail her at djohnson@capousd.org, if you have any questions.
