CROWN
VALLEY ELEMENTARY
SCHOOL TOURS
The administration of Crown Valley
will be conducting tours of the campus for families new to the area or the
school. These tours are scheduled the third Tuesday of the month; February
19, March 18, April 15 and May 20. If you are interested in joining a tour
group, please contact Crown
Valley’s office

Information
To register
a child for Kindergarten for the 2008-2009 school year, the child must be 5
years old before December 2, 2008. The documents necessary for registration
include:
ü Legal Birth Certificate, Passport, or
Baptismal Certificate
ü Proof residency (2 forms needed)
o Current utility bill
o Rental/mortgage agreement
ü School Entry Physical (dated March 2008
or later)
ü Verification of current immunizations
ü Completed CUSD Registration Packet
o Pupil Admission Form
o Blue Immunization Form (parents must
complete all information on card)
o Buff Health Record Card (parents
complete left hard portion of card)
o Ethnicity Survey
o Home Language Survey

If
home language is other than English, parents will be given a “ticket
to test” at CUSD’s Language
Acquisition Center
ü Other documents:
o Kindergarten Questionnaire
o Placement Request (AM/PM sessions).
This request does not guarantee placement.
o Transportation pre-registration (if
necessary)
ü $5/one-time donation to Crown Valley’s Disaster Fund
(payable to Crown Valley PTA)
GRADES 1-5 REGISTRATION
The
