What to bring for registration
1. TWO current proof of residency:
Mortgage Document or
Rental/Lease Agreement and
Utility Bill (water, gas or electric)
2. Proof of immunizations:
Yellow card from Health Department
or record from Doctor’s office or clinic.
3. Date of Birth Verification:
Birth certificate, baptismal record
or passport.
Please note:
Registration packets will be accepted only if all immunizations are current and immunization, address, and date of birth verifications are provided.
