How do I Obtain a Copy of My Immunization Record?

For current students or students that graduated last school year, please contact your High School.

For students that attended/graduated previous to the last school year, please send the information listed below to:

Please include:

• Your name at the time of graduation
• Date of Birth
• Year graduated or last year attended
• High School graduated from or last school attended
• Name and address of where to send the Immunization record
• Check or money order made out to CUSD – ($5.00 fee per copy)

Requests for Immunization Records need to be made by the student. Once you become 18 years of age, you must authorize the release of your Immunization record. Please be sure to sign your request.

How Do I Obtain A Copy of My Transcripts?

For students that attended/graduated in 1997 to the current school year, see below for instructions on how to order your transcripts online.

(Transcript fees for current graduates are $10.00 per copy and alumni are $15.00 per copy)

For students that attended/graduated in 1996 or previous to that year, please send the information listed below to:

Requests for High School Transcripts need to be made by the student. Once you become 18 years of age, you must authorize the release of your High School Transcript. Please be sure to sign your request.

If you have lost or misplaced your High School Diploma, we do not issue duplicate copies. The official legal document to show proof of graduation is the High School Transcript.
