The Capistrano Unified School District must be named as additional insured and certificate holder on the Certificate of Liability Insurance form Acord 25.

 

A separate endorsement page is required and should include your policy number and name the Capistrano Unified School District as an additional insured.

 

Blanket endorsements are not acceptable.

 

Required Endorsement:
 
The Capistrano Unified School District is named as additional insured.  Such insurance as is afforded by this policy shall be primary, and any insurance carried by DISTRICT shall be excess and noncontributory.

General Liability Certificates (Consultants and Independent Contractors)
Required Coverage Amounts:
$1,000,000 minimum limit per occurrence
$2,000,000 minimum general aggregate

 

Workers Compensation
Workers Compensation coverage of $1,000,000 per occurance is required for any independent contractor/consultant that employ one or more persons.

A Waiver of Subrogation is required on the Workers Compensation endorsement.

If you do not employ anyone in the manner subject to the worker's compensation laws of California complete a Certification Form

Professional Liability (Errors and Omissions) Policies
Can be substituted for general liability policies.
Does not require CUSD to be named as a Certificate Holder and Additional Insured
$1,000,000 minimum limit per occurrence
$2,000,000 minimum general aggregate

 

Construction Contractors
Refer to your agreement with CUSD to determine which coverages are required.
